3. Electronic health record

Definition
Is an electronic record of health-related information.

How Used in Healthcare
Electronic health records are similar to are used to store patient information. Electronic health records can contain information about the patient’s medical history, their demographics, their general health, their past and present medical conditions, treatments received, examination results, prescribed medications, doctor’s notes, and more.

4. Personal health record

Definition
Is a personal health care record created by the patient themselves.

How Used in Healthcare
• A personal health record is information a patient creates for themselves. Information my include; patient’s name, birth date, blood type, emergency contact, date of last physical, dates and results of tests and screenings, any illnesses and surgeries; along with dates , list of your medications, any allergies, any chronic diseases, and any family history of illnesses.

5. Computerized provider order entry system

Definition
Computerized physician order entry is the process of entering medication orders electronically instead of using paper charts.

How Used in Healthcare
Computerized provider order entry refers to any system in which providers input prescription orders directly into a computer system. These systems have become common and it is used mainly because it reduces medication errors.

6. Unique patient identifier

Definition
The identity of an individual that consists of a set of personal characters that individual can be recognized.

How Used in Healthcare
Identification is the proof of one's identity. Patient Identifier is the value assigned to an individual to facilitate positive identification of that individual for healthcare purposes. Unique Patient Identifier is the value permanently assigned to an individual for identification purposes and is unique across the entire national healthcare system. Unique Patient Identifier is not shared with any other individual, (Appavu, 1997).
Where I work, we use a patient’s name (first and last), ID card, date of birth, the last four of the sponsor’s social security number and their middle initial to identify a patient. We have to use at least 3 identifiers per patient.

7. Protected health information

Definition
Protected health information includes any individually identifiable health information.

How Used in Healthcare
